Family Connections is available to families in San Mateo County who qualify as low income.
Applying is easy, and all qualified families are either accepted directly into our program or placed on our waiting list. Tuition is free.

Complete Registration Forms
Forms are available from the teacher. Below is the information needed to complete the registration process. Note: additional documentation may be required.
Submit the following additional documents to complete registration:
1. A copy of your Child's Immunization Record
2. A copy of the Participating Adult's (Parent and/or Guardian) TB Test Results
3. Emergency Contact information card
4. Monthly Income (this information will be kept confidential)
